Diabetes Management Program
UConn Health’s Diabetes Self-Management Education Program is designed to help patients living with diabetes understand the disease and learn to manage it. Our program, led by Certified Diabetes Care and Education Specialists, has been recognized by the American Diabetes Association for meeting its national standards for self-management education programs.
Kidney Disease Education Program
As the number of patients living with kidney disease increases, it is important to understand the ways to manage and live with the disease. The Kidney Disease Education Program at UConn Health helps patients understand how the kidneys work, how a renal diet may help, how to understand laboratory values, what lifestyle changes can help prevent dialysis, and which treatment options are available to help slow kidney function loss.
